Due to climate reasons, the quantity of epidemic hemorrhagic fever cases in Shaanxi province has been elevated in 2021 compared with previous years.On the afternoon of January 26, 2022, Seminar on Treatment of Hemorrhagic Fever in Midwest China, organized by East District of the First Affiliated Hospital of Xi 'an Jiaotong University (XJTU),was held online and offline in Yanliang, aiming to timely summarize the treatment experience of hemorrhagic fever,enhance the academic exchange level, popularize the latest treatment experience, and provide reference for the treatment of hemorrhagic fever in the midwest regions and even the whole country.
More than 20 experts in the fields of infectious diseases and critical care medicine from Shaanxi province and beyond were invited to gather at the seminar. At the opening ceremony, Lyu Yi,President of the First Affiliated Hospital of XJTU, Liu Feng, Director of Shaanxi Administration of Traditional Chinese Medicine and Shaanxi Provincial Center for Disease Control and Prevention, and Yang Ruiping, Director of Yanliang District Health Bureau delivered opening speeches, respectively. The opening ceremony was chaired by Zou Yuliang, Executive President of East District. During the seminar, experts shared and exchanged ideas regarding the highlights of early diagnosis of hemorrhagic fever, treatment of critical hemorrhagic fever patients, hemorrhagic mechanism and prevention of hemorrhagic fever and acute renal injury induced by hemorrhagic fever,etc. At the seminar, East District, as the designated treatment affiliation of hemorrhagic fever in Shaanxi province and seminar organizer, shared the achievements and treatment experience of this round of hemorrhagic fever.
Facing the dual threats of COVID-19 pandemic and hemorrhagic fever,our hospital carried out unified arrangement,coordinated two hospital districts, held several coordination meetings for hemorrhagic fever treatment, and established hemorrhagic fever treatment team with Department of Infectious Diseases, Department of Emergency and Critical Care Medicine and Department of Nephrology as the core, aiming to guarantee the successfultreatment of hemorrhagic fever patients during COVID-19 pandemic.
In October, 2021, Professor Wang Xue from Department of Critical Care Medicine of our hospital, a prestigious expert in critical care medicine across China, led a medical team to settle in East District, integrated the resources of Department of Emergency Medicine, First Aid Station and Department of Critical Care Medicine in East District, and established Department of Emergency and Critical Care Medicine with the goal of effectively treating acute and critical patients and began to receive hemorrhagic fever patients. This year,the number of patients with critical and severe hemorrhagic feverwas increased, which posed a challenging threat to medical staff.
A 31-year-old male patient progresse drapidly into a critical condition upon admission. Traditional treatment failed to achieve clinical reversal, and ECMO was the last option. The team quickly responded and closely cooperated to offer ECMO forthe patient. After ECMO treatment, vital signs of this patient were immediately improved. After timely treatment and meticulous nursing care fromm edical staff, the patient's condition was gradually improved and discontinued ECMO management on the 3rdday. He is also the first case of severe hemorrhagic fever who successfully discontinues ECMO without any complications in Shaanxi province.
At the beginning of December,the number of hemorrhagic fever cases was persistently increased in Shaanxi province. Under the unified deployment of the leadership team of our hospital, East District undertook the important task of treating patients with hemorrhagic feveras a designated affiliation in Shaanxi province. The hospita l rapidly set up a medical team and established a multidisciplinary medical team for hemorrhagic fever. The main hospital organized amedical team of 6 specialists including Chief Physician Fan Wanhu from Department of Infectious Diseases to settle in East District. Under the leadership of Director Liu Hua, Department of Nephrology gave full play to disciplinary characteristics and delivered symptomatic treatment. Department of Emergency and Critical Care Medicine spared no effort to treat critically ill patients. To guarantee the successful treatment of patients with hemorrhagic fever, the hospital rapidly carried out emergency reconstruction in partial areas of Departmentof Emergency Medicine and allocated 15 wardbeds and related instruments and facilities within 1 week, making full preparation for the treatment of more patients.
Up to now,our hospital has treated 205 patients with hemorrhagic fever,and 50.7% of them were critical and severe cases.The main hospital received 32 patients and 173 cases were admitted to East District. Throughout the treatment of patients with hemorrhagic fever, two hospital districts delivered multidisciplinary cooperation, which not only contributed the "regimen of the First Affiliated Hospital of XJTU" for the treatment of hemorrhagic fever, but also highlighted the strategic significance of the layout of multi-hospital districts during COVID-19 pandemic, building a solid fortress safeguarding the health of people with responsibility and expertise.
