According to the 2024 National Cancer Report released by the National Cancer Center of China, China recorded 4.8247 million new malignant tumor cases in 2022, accounting for 24.1% of the global total. Cancer deaths exceeded 2.5742 million, representing 26.5% of global fatalities. China has become a veritable “cancer superpower.” Regarding colorectal cancer, the incidence rate in the United States (46.1 per 100,000) exceeds that of China (36.63 per 100,000), yet the mortality rate is lower in China (17.00 per 100,000 vs. 15.8 per 100,000 in the U.S.). The incidence-to-mortality ratio stands at 2.9:1 in the U.S. versus 2.15:1 in China. This indicates that despite higher detection rates among colorectal cancer patients in the U.S., treatment outcomes are superior, yielding better survival rates. The higher incidence-to-mortality ratio in the U.S. underscores the critical importance of early screening, standardized comprehensive diagnostic and therapeutic techniques, and holistic management throughout the disease course.
Natural Orifice Specimen Extraction Surgery (NOSES) is a specialized minimally invasive surgical technique hailed as “the least invasive of minimally invasive procedures.” It enables the removal of lesions without leaving noticeable scars, thereby reducing patient pain and psychological burden. This technique has been applied across multiple fields including colorectal surgery, gastric surgery, hepatobiliary and pancreatic surgery, gynecology, urology, thoracic surgery, and thyroid surgery. Over 120,000 NOSES procedures have been performed domestically, gaining increasing popularity among both physicians and patients. To promote this technology, Professor Wang Xishan from the Cancer Hospital of the Chinese Academy of Sciences spearheaded the establishment of the International NOSES Alliance and the Chinese NOSES Alliance. Provincial branches have progressively been formed, hosted regular training sessions and competitions. Surgical skills are enhanced through live online surgeries and video sharing. Furthermore, the technique has gained international recognition, being translated into multiple languages to foster global academic exchange. In summary, the development of NOSES minimally invasive surgery represents an inevitable advancement in medical progress. It not only enhances surgical efficiency but also significantly improves the patient experience. As the technique becomes more widespread and surgeon proficiency increases, NOSES surgery will play an even greater role in the future, bringing benefits to more patients. However, the NOSES procedure is complex, technically demanding, involves numerous detailed steps, and requires strict adherence to aseptic and tumor-free operating protocols. Surgical execution demands precision and uniformity, necessitating “systematic” training. Structured training thus becomes paramount, enabling systematic theoretical knowledge review and step-by-step standardized practice for trainees.
To further internationalize the NOSES minimally invasive surgical technique, achieve global standardized skill training for surgeons, strengthen the scientific rigor, standardization, and systematic approach of treatment, and elevate medical standards across different regions and tiers of healthcare centers. Hosted by the International NOSES Alliance and the China NOSES Alliance, and organized by the Shaanxi Branch of the China NOSES Alliance and Xi'an Jiaotong University First Affiliated Hospital—leveraging the National Medical Center and the National Clinical Skills Experimental Teaching Center—this initiative integrates premium medical education resources to launch the International NOSES Structured Training and Certification Program. This course aims to disseminate scientifically standardized minimally invasive NOSES techniques for gastrointestinal tumor diagnosis and treatment through systematic training, providing technical assurance for improving patient outcomes—particularly for colorectal cancer patients. Through standardized procedures, quality control, and faculty development, participants will progress from awareness to operation, proficiency, and mastery. Conducted academic visits at Yale University School of Medicine from 2017 to 2018. Clinic hours: Tuesday afternoons and Friday mornings. Dr. Liu has edited and co-authored 20 professional publications including “Theory and Practice of Minimally Invasive Surgery for Colorectal Cancer.” He has led 12 national/provincial research projects and published over 100 papers in SCI and core journals, focusing on molecular biomarker screening and prognostic model development for colorectal cancer. In 2023, he performed 3 robot-assisted surgeries, 9 natural orifice specimen extraction surgeries (NOSES), completed 12 complex procedures, and participated in 108 multidisciplinary consultations. He proposed a differentiated application strategy for fecal immunochemical testing (FIT) and colonoscopy in colorectal cancer screening. He chaired the International NOSES Classic Surgery Video Competition at the 17th National Colorectal Cancer Academic Conference and delivered specialized lectures on comprehensive treatment for peritoneal metastasis in colorectal cancer across multiple regions.
