Department of Neurology completes the first case of FRED flow diversion device for the treatment of intracranial aneurysm in northwest China

On June 1, 2023, the team led by Professor Han Jianfeng and Professor Yu Jia from Department of Neurology of the First Affiliated Hospital (FAH) of Xi’an Jiaotong University (XJTU) first applied novel FRED flow diversion device to successfully treat an intracranial aneurysm patient, and completed the 1st case of FRED flow diversion device in northwest China and the 4th case in China, signifying that the neurointerventional team from Department of Neurology in the FAH took the leading position in the interventional treatment of intracranial aneurysms in northwest China and ranked among the leading group in China.

A 54-year-old female patient reported chief complaint of intermittent dizziness for 2 months. CTA of the head and neck in a local hospital showed an aneurysm located in C5 segment of the left internal carotid artery, and subsequent DSA confirmed that the aneurysm was approximately 3mm×3mm in size. After admission, explicit preoperative examinations, comprehensive preoperative consultations and adequate drug preparations were carried out. Professor Han Jianfeng and Professor Yu Jia led the team members including Liu Fude, Sun Peng and Chen Chen, to implant FRED flow diversion device for the patient under general anesthesia with tremendous support from Department of Anesthesiology and Perioperative Medicine and Interventional Operating Room. The surgical procedure was successful and satisfactory surgical results were immediately obtained. Postoperatively, the patient was transferred to ICU of Department of Neurology for intimate observation, and transferred to the general ward the next day. She was scheduled to be discharged after recovery.

In recent years, endovascular treatment has become one of the major therapeutic options for intracranial aneurysms due to few complications and low mortality rate. Flow diversion device is a new-type interventional treatment material, which represents the changing treatment concept from intracapsular packing to reconstruction of tumor-bearing artery and brings novel ideas and methods for endovascular treatment of complex aneurysms. Compared with alternative flow diversion devices, FRED flow diversion device adopts unique double-layer design. The conventional mesh design of the outer layer can improve the delivery performance of the stent and reduce the coverage of normal branches. Meantime, the dense mesh structure design of the inner layer can better exert the hemodynamic effect, which has been proven to yield high safety and efficacy in multiple clinical trials worldwide.
