Department of Neurology completes the first case of Kylin flow-diverter device for intracranial aneurysm in northwest China

On September 9, 2024, the team of Vice President Han Jianfeng and Professor Yu Jia from Department of Neurology in the First Affiliated Hospital (FAH) of Xi’an Jiaotong University (XJTU) successfully performed surgery using Kylin flow-diverter device for a patient with intracranial aneurysm for the first time, which is the first case of clinical application of Kylin flow-diverter device in northwest China, demonstrating that the neurointerventional team from Department of Neurology of FAH is in the leading position in the interventional treatment of intracranial aneurysms in northwest region and China.

A 60-year-old woman developed intermittent dizziness for 2 months. CTA of the head and neck at a local hospital showed an aneurysm in the C6 segment of the left internal carotid artery, and DSA confirmed that the aneurysm was approximately 7.9×4.9 mm in size. She was admitted to Department of Neurology of FAH for further surgical treatment.

Upon admission, comprehensive preoperative examination and consultation, and drug preparation were carried out. Professor Han Jianfeng and Professor Yu Jia led the team members to perform the surgery. With the support from Department of Anesthesiology and Perioperative Medicine and Interventional Operating Room, a Kylin flow-diverter device was placed into the patient under general anesthesia. The operation process was smooth and the stent was properly attached to the wall. Postoperatively, the patient was transferred to ICU of Department of Neurology for intimate observation, and transferred to the general ward the next day. Currently, she is discharged after recovery.

The principle of flow-oriented dense mesh stent in the treatment of intracranial aneurysms is as follows: local blood flow can be reshaped through the design of high metal coverage and high mesh rates of the flow-oriented dense mesh stent, and the blood flow from the parent artery into the aneurysm is directed into the distal normal vessels, which can reduce the impact of local blood flow on the aneurysm, improve hemodynamic status within the aneurysm and finally form a thrombus within the aneurysm, thereby realizing the occlusion of the aneurysm. The advent of dense mesh stent also revolutionizes the treatment strategy of aneurysms from intracapsular tamponade to parent artery remodeling, which brings novel ideas and methods for endovascular treatment of complex aneurysms. Currently, Kylin stent is the only flow-diverter device with nickel-titanium coated platinum and distal closed structure. The super-elastic nickel-titanium material makes the stent fully adhere to the wall of different vessels, and the distal end is closed, which causes mild injury to blood vessels and is more convenient to treat distal occlusion. On the other hand, the “macaroni” technology is employed to manufacture nickel-titanium coated platinum, which can achieve whole-body development. With the assistance of three Mark points at the distal and proximal ends, intraoperative positioning can be performed more precisely, which has demonstrated high safety and efficacy in clinical application.
