Clinical case from Department of Vascular Surgery is selected as "The Most Valuable Cases of the Year of the 14th China Endovascular Course"

Recently, the case reported by Department of Vascular Surgery of the First Affiliated Hospital of Xi’an Jiaotong University (XJTU) was successfully selected as "The Most Valuable Cases of the Year of the 14th China Endovascular Course". China Endovascular Course (CEC 2022), hosted by Vascular Surgery Branch of Chinese Medical Doctor Association, is the most influential international annual meeting of vascular surgery. A total of 49 international scholars, 464 domestic experts and 293 guests participated in CEC 2022.

"The Most Valuable Cases of the Year" event is always the highlight of CEC each year. Only complicated and difficult cases can be selected from regional preliminary and final competitions, which have the highest teaching significance. Atherosclerotic occlusive disease of the lower limbs often involves the common branches of the superior and inferior genicular arteries, leading to severe ischemia and even amputation of the lower limbs, which is a health-threatening condition for patients. Under the guidance of Director Lu Shaoying and Academic Leader Liu Jianlin in the preliminary stage, difficult and representative clinical case of atherosclerotic occlusive disease of the lower limbs was selected to showcase clinical diagnosis, treatment and surgical levels of Department of Vascular Surgery.

The patient was diagnosed with toe ulcer. Preoperative CTA showed alumen occlusion segment of approximately 32cm in length in the superficial femoral artery complicated with long segment occlusion of the anterior and posterior tibial arteries. The patient was also complicated with a large area of right toe ulceration, which should be treated by amputation. It was a TASC/D lesion in the superior and inferior genicular arteries, and it was challenging to simultaneously treat the occlusion of superior and inferior genicular arteres. Before the surgery, Associate Research Fellow Wang Jichang cooperated with Physician Li Yan and Physician Jia Yifan to carefully determine surgical plan and coping strategies for unexpected situations during the surgery, and delivered in-depth discussion with Director Lu Shaoying and Academic Leader Liu Jianlin. After full preparation, the surgery was successfully started. The occlusion of superficial femoral artery and anterior tibial artery responsible for the ulcer was simultaneously treated. At 3-month outpatient follow-up, the ulcer was completely healed, suggesting the treatment was highly efficacious.

Based on difficult case and high postoperative efficacy, Associate Research Fellow Wang Jichang grabbed the honorary title of "The Most Valuable Cases of the Year", fully demonstrating appropriate preoperative planning, surgical expertise, attentive perioperative management skills and intriguing presentation demeanor of Department of Vascular Surgery team in our hospital, receiving widespread recognition from the experts.
