On the morning of October 10, 2024, under the guidance of Professor Yuan Zuyi, President of Cardiovascular Disease Hospital, Professor Guo Ning, Associate Professor Zhang Yong and the team from Department of Cardiovascular Medicine cooperated with Department of Anesthesiology and Perioperative Medicine and Interventional Operating Room to successfully perform the first case of renal denervation (RDN) for a patient with refractory hypertension in Shaanxi province.

This operation is the first case of RND in Shaanxi province after the expert team from Department of Cardiovascular Medicine in the First Affiliated Hospital (FAH) of Xi’an Jiaotong University (XJTU) performed RND in March, 2024 with the support of the "early and pilot implementation" policy of Hainan Boao Research Hospital.
The 41-year-old male patient had a family history of hypertension complicated with diabetes mellitus and coronary atherosclerosis. At 2 years after the diagnosis of hypertension, the 24-h average ambulatory blood pressure remained at 148/99 mmHg after regular intake of dual antihypertensive drugs, suggesting that the blood pressure is not properly controlled. After preoperative screening, the risk of secondary hypertension was excluded. After comprehensive evaluation of renal function and renal artery, it was determined to perform interventional RDN for hypertension, aiming to effectively control the blood pressure and reduce the risk of cardiovascular and cerebrovascular adverse events.
The most advanced Symplicity Spyral multielectrode RDN catheter was adopted during the procedure. With intimate cooperation from Attending Physician Yang Tingting, an anesthesiologist, the operation was completed painlessly and effectively within 1.5 h. Intraoperatively, 33 targets of the main and branch arteries of bilateral kidneys were ablated, achieving complete ablation of the renal arteries. RDN procedure was successfully completed.

RDN is a novel procedure for the treatment of refractory or complex hypertension. It is a minimally invasive interventional procedure with the characteristics of no implantation, rapid recovery and high efficacy. In the early 2024, the teams led by Professor Yuan Zuyi and Professor Guo Ning performed RDN using Symplicity Spyral ablation system for a patient with refractory hypertension in Boao pilot zone, Hainan province. Postoperatively, the blood pressure and renal function were significantly improved. RDN can better control the blood pressure, and achieve the effects of continually lowering blood pressure, reducing fluctuations and decreasing drug burden in hypertension patients.
