According to the source news from Yulin Hospital, the First Affiliated Hospital (FAH) of Xi'an Jiaotong University (XJTU) on March 29, 2024, the hospital recently adopted variable-diameter balloon-expandable stent to successfully perform surgery for a case of iliac artery pseudoaneurysm complicated with internal iliac aneurysm. After searching the Sci-tech Novelty Retrieval Workstation of the Ministry of Education, this surgery is the first of this kind worldwide.

The 73-year-old patient, surnamed Bai, was diagnosed with cerebral infarction and grade 3 hypertension. CT scan revealed a huge pseudoaneurysm in the right iliac artery complicated with the right internal iliac aneurysm, which was a life-threatening condition with a high risk of rupture.
Professor Liu Jianlin's team from Department of Interventional Vascular Surgery in Yulin Hospital of FAH of XJTU admitted the patient and considered that the elderly patient was diagnosed with cerebral infarction complicated with grade 3 hypertension (extremely high-risk grade). Surgical risk was extremely high if the aneurysm were surgically removed and the iliac artery was repaired or replaced by artificial vessels. If conventional interventional procedure was adopted to isolate the pseudoaneurysm with covered stent, the risk of internal leakage will be high for patients with significant differences between the diameter of common iliac artery opening and external iliac artery, thereby leading to surgical failure. Consequently, Liu Jianlin's team decided to choose variable-diameter balloon-expandable stenting surgery.

The surgery was successfully completed enduring for 90 min. Postoperative angiography showed that the position of covered stent was accurate after diameter variation, which completely covered the rupture of the right common iliac artery pseudoaneurysm and the opening of the right internal iliac artery. No internal leakage occurred. Postoperatively, the patient's vital signs were stable, and was discharged after 2 d.
