The world's first case of endoscopic pulsed field ablation (PFA) for tumors successfully completes

Recently, Department of Hepatobiliary Surgery successfully performed the world's first case of endoscopic pulsed field ablation (PFA) for a patient with locally advanced pancreatic cancer. The successful application of this innovative medical technology is another achievement in the field of engineering medicine by Xi 'an Jiaotong University (XJTU), indicating that China has made a major breakthrough in the innovative technology of endoscopic surgery, which is expected to provide a novel diagnosis and treatment approach for hepatobiliary and pancreatic tumors.

This patient was diagnosed with locally advanced pancreatic cancer, complicated with obstructive jaundice, diabetes mellitus and other primary diseases. He failed to tolerate chemotherapy due to poor physical condition and conventional treatment could not be effectively adopted. According to the patient's condition, Professor Lyu Yi, Vice President of XJTU, delivered preoperative consultation with other experts from Department of Hepatobiliary Surgery, Department of Biliary-Pancreatic Endoscopic Surgery and Department of Pancreatic Surgery of the First Affiliated Hospital of XJTU, frequently communicated with the patient and finally decided to perform endoscopic PFA. After full preoperative preparation, with the support and cooperation from Department of Anesthesiology and Perioperative Medicine, Interventional Operation Room and other teams, Chief Physician Sun Hao, Deputy Chief Physician Yang Xue, Attending Physician Li Yu and M.D. Ren Fenggang from Department of Hepatobiliary Surgery successfully performed ERCP+ endoscopic PFA +biliary stent implantation for this patient. Due to high energy density of pulsed electric field, the pulsed ablation process only took approximately 100 seconds, and the average temperature in the ablation area was 40℃. The surgical process was safe and controllable. Postoperatively, the patient obtained stable vital signs, restored bile drainage and achieved effective recovery.

According to the latest literature review, this is the world's first case of endoscopic PFA for pancreatic tumors. Domestic high-performance medical devices with completely independent intellectual property rights were adopted in this surgery. EndoPulse HPB array pulsed electric field ablation catheter is independently developed by Professor Lyu Yi's team and transformed into clinical products by Medneering Healthcare. The head end of this catheter is composed of multiple ring-shaped electrodes, which can meet the requirements for different types of lesions. Dophi® steep-pulsed device is adopted as the pulsed field energy platform, which is jointly developed by SURGNOVA and Professor Lyu Yi's team. The pulsed field device is the second device certified by CE worldwide, which significantly enhances the intelligence and integration capability compared with international products.
